Frankly, I don't understand what you mean by signature. There are 2 places to enter names in the written text. Which one should I write in blood or should I write my name in a different place? Can you please explain in detail?

Separately from the prayer you sign your name in blood like you would sign a letter or something, just with blood. But your fingerprint also does it, if you can't get that much blood.
A tip for those that don't want to use a sewing needle or pin. Use a glucose tester. they are easy to pick up in a grocery store pharmacy section and if someone asks you about it just say your picking it up for a friend. those things are built literaly to help you get blood from your fingertips and as it's been pointed out you can just do a thumbprint which doesn't require much. but the tester make sure you don't stick yourself too far down into your finger and it hurts less.
