Does one individual or a group of people decide if a posting or reply is put on the forum.

Yes, the mods do need to approve.

Administrators and moderators approve and disapprove posts and take care of handling reported posts.

Then there is a group of people that do get to post freely without approval because of their track record. But, if this freedom is abused, it can be taken away. And their posts just like everyone elses can be reported and deleted.
