E.g if Saturn is conjunct Almach in the 8th, would this counteract the positive description of Almach and give the complete OPPOSITE meaning? Or is it more like with some effort/difficulty this thing can happen

Everything in the chart needs work and investment to manifest, unless you wish it to manifest in a wrong way or too small. Whether good or bad star this all needs hard work and investment to fully benefit you, especially the one conjunct Saturn. Despite of a star Saturn needs realization through building anyway. The worse star / sign, the more work. There are some charts where Saturn and stars connected to planets are full harmonic and beneficial but those people who do not work with them, will not extract the real potential of them, for example good thing will happen once or twice in life on its own, but they could make it their permanent fate and use it to invest into JoS and vastly accelerate their own advancement and the advancement of the whole country or continent, or the world. But if they do not work on it it might subside with time. Good things need to be developed to not subside.

8h Saturn with Almah need to be worked upon to achieve great influence and power, including spiritual and financial one. Saturn gives only those benefits that are achieved by hard labor and building things from zero. Almah might offer good reward for hard personal efforts.
