I've got a very odd problem. I'm in my late 20's and obviously I still have no problem getting an erection and holding it for a very long time, but once I start penetration - it starts getting soft right away and then refuses to go up for a long time as if I just ejaculated.

A too tight condom might restrict blood flow, which may cause softness during intercourse. You might want to consider trying thinner or better-fitting condoms to improve sensation and arousal. Performance anxiety could also be a contributing factor to the difficulty in maintaining an erection.
